Care Plan from Journey Forward
According to
Journey Forward, a new program designed for doctors and their patients who have recently completed active treatment for cancer, a Survivorship Care Plan "is a coordinated post-treatment plan among the Survivor's oncology team, a primary care physician and other health care professionals. The oncologist creates a summary of the Survivor's treatment and includes direction for future care."
The Journey Forward website provides a downloadable Survivorship Care Plan, which includes:
- Patient diagnosis and treatment summary
- Best schedule for follow-up tests
- Information on late- and long-term effects of cancer treatment
- List of symptoms to look for
- List of support resources
Journey Forward also offers several other helpful tools, including Tips on Talking to Your Doctor, A Medical History Builder, a Resource Directory and a Survivorship Tool Kit.
