Care Plan from ASCO

The ASCO (American Society of Clinical Oncology) Cancer Treatment Plan and Summary is a brief record of your cancer treatment. The forms provide a convenient way to store information about your cancer, cancer treatment, and follow-up care.

The Breast Cancer Survivorship Care Plan was developed to help doctors and their patients develop a customized follow-up care plan once treatment is nearing completion. The recommendations for follow-up tests are based on the best available scientific evidence from ASCO's clinical practice guidelines, which are summarized in the What to Know: ASCO's Guidelines.
